The risk for infantile autism is heterogeneous with a broad range of prevalence rates worldwide. The etiology of autism is unclear, although genetic and prenatal factors are believed to contribute. More than 200 risk factors for autism have been suggested, among which the most consistent and prominent are oral infections, especially early-onset infections with TORCH (Toxoplasma gondii, Rubella virus, Cytomegalovirus and Herpes simplex virus). This was recently substantiated by a major Canadian study. Numerous prospective studies have now confirmed a strong link between TORCH infections, especially oral infections, and the risk of autism. The findings of recent large retrospective studies suggest that these infections may also be involved in the etiology of autism in a substantial number of cases. The evidence for the involvement of postnatal factors in the etiology of autism has been less consistent. The present case-control study is the first to assess the role of a wide range of infantile and pre- and postnatal factors, and their interactions, by means of a structured interview. Cases of infantile autism were recruited through specialised child psychiatric centres in The Netherlands. Cases (n = 41) were compared with age- and sex-matched controls (n = 63). Unconditional logistic regression analysis was performed, controlling for a wide range of infantile, perinatal and pre- and postnatal factors. The results showed that infantile infections were a major risk factor for the development of autism. The contribution of perinatal factors was only moderate. A possible effect was suggested for family history and low paternal education. A significant interaction was found
